SEATTLE PUBLIC SCHOOL FORMS

SPS requires certain forms to be completed annually for each of your children:

  • Emergency Information and Student Release

  • Student Health Information

  • FERPA (Family Educational Rights and Privacy Act)

  • Authorization for walking field trips

 

Please follow these steps to fill and submit:

  1. Download fillable PDF of all four required forms: HERE

  2. Complete all pages of the PDF, including all electronic signatures.

  3. Save PDF file to your computer with the name "SPS Required Forms" and your student's first and last names (example: SPS Required Forms - Jack Snow)

  4. Email completed PDF to: jsis.forms@seattleschools.org
